Identifiable information about you will only be shared in the situations explained below.

When information about you may be shared
  • To support your care - for example with district nurses or hospital services.
  • To help you access other services - for example social care support, with your consent.
  • Where there is a legal duty to share information - for example child protection cases.

In some cases, anonymous patient information may also be used to help the Health Board or Government plan services, such as diabetes care. This information does not identify you.

If you do not wish for even anonymous data about you to be used in this way, please inform us.

Welsh GP Record

What is the Welsh GP Record?

The Welsh GP Record is a summary of key information from your full GP medical record. It is accessible to authorised health professionals in Out of Hours services and some Secondary Care settings.

This summary may include:

  • Your name, address and contact details
  • Current medication and prescriptions from the last two years
  • Any allergies or adverse reactions
  • Current diagnoses or health problems
  • Results from recent tests within the past year, such as blood tests or X-rays

Please note: it does not include private discussions with your GP.
