Welcome to Amman Gwendraeth Cluster
There are seven surgery sites and their partners within the Amman Gwendraeth Cluster:
Cluster vision
- To develop an integrated system of primary, community and social care.
- Patients will be able to flow through the sectors as needed during their journey, based on pathways for different conditions.
- Supporting our local population to remain in their own home, with an emphasis on population wellbeing and community connection by establishing greater links with partner services.
Multidisciplinary team working
We have regular meetings in General Practice with our district nursing, occupational therapists, physiotherapists, and social workers and other important teams to co-ordinate care in our community for our older population.
We also have regular meetings with our mental health and palliative care colleagues.
Pharmacists in practice
We have pharmacists working within our surgeries throughout the cluster as part of the general practice team.
